Hazel May Hoffman

Hazel May Hoffman, 85 of Collinsville, OK passed away Friday, November 7, 2008 in Owasso, OK. She was born January 12, 1923 in Tulsa, OK to Therman Grigsby and the former Lillie Simmons. She had lived in Collinsville since 1966; she was a member of the American Legion and the Moose Lodge.
She is survived by 2 sons:, Michael Hoffman of Tulsa, OK and Ron Hoffman and his wife, Cynthia of Tulsa, OK. 1 Daughter: Jeanne Worell and her husband Ray of Wimberly, TX. She is preceded in death by her husband Michael J. Hoffman. 1 son: Terry Lee Stoffel and 1 daughter: Patricia Ross.
